We welcome referrals from primary care clinicians, therapists, hospitals, assisted living communities and memory care programs.

We provide psychiatric evaluation and medication management for adults with complex mental health needs.

• Ongoing updates when clinically appropriate

Continuity Matters

When a patient's condition is stable, medication management may transition back to the primary care clinician when appropriate.
